Are you planning on selling prepared-on-site food items? *

If you are planning on selling food items, we will need a bit more information.

We need you to describe your menu in the box below. Please be sure to list any items you are planning on selling. We do this to try and eliminate overlap and duplication of food items. We may contact you prior to the event to let you know if you cannot sell a particular item and work with you to find a replacement. If you do not list an item here, you will not be allowed to sell it at the event.

Food vendors require approval before being allowed to register at our event. Our vendor coordinator will be in contact with you to confirm your reservation. *

I understand that registration for food vendors is CONDITIONAL. Although I am registering, I recognize I am not guaranteed a location at the event. If i am not approved, my application fee will be refunded.
I understand that participation in our show is conditional on being approved by our vendor coordinator
I understand that I must comply with all regulations from the Oklahoma State Dept. of Health regarding food preparation, sanitation and other requirements

For food vendors, we assess an additional $15 surcharge for utilities per booth.
